Amylase - Body Fluid

An amylase test measures the enzyme amylase in blood or other body fluids (urine, pleural or peritoneal fluid, sometimes CSF) to help detect pancreatic or salivary gland injury, acute pancreatitis, pancreatic leaks, or gastrointestinal perforation. Elevated amylase in a fluid suggests pancreatic or salivary origin; comparing fluid and serum levels helps identify pancreatic leakage or pancreatic ascites.

Normal amylase in drain fluid is low—typically similar to serum amylase and generally less than about 300–350 U/L. Clinically, an amylase level in drain fluid exceeding roughly three times the patient’s serum amylase (commonly assessed on or after postoperative day 3) suggests a pancreatic leak or fistula and should prompt further evaluation and management.

For serum amylase: have venous blood drawn into a plain or serum‑separating tube by venipuncture; avoid hemolysis and send to the lab promptly. For urine: collect a clean‑catch midstream or a 24‑hour specimen in a sterile container, refrigerate and deliver quickly. For saliva or peritoneal/pancreatic fluid, use sterile containers and aseptic technique (clinician‑performed for invasive collections). Label and note collection time.

Amylase is present in saliva (produced by salivary glands) and pancreatic secretions (pancreatic amylase). It also appears in blood (serum/plasma) and is excreted in urine. Trace activity can be detected in human breast milk. Clinical testing measures serum and urine amylase to evaluate pancreatic disorders; salivary amylase aids initial carbohydrate digestion in the mouth.

Elevated amylase commonly indicates pancreatic inflammation (acute or chronic) but can also result from salivary gland disease, gallstones, abdominal injury, certain medications, renal failure, or macroamylasemia. Symptoms include upper abdominal pain, nausea, and vomiting; severe cases need urgent care. Clinicians usually repeat tests, check lipase, and use imaging (ultrasound/CT) to identify the cause and guide treatment—seek prompt evaluation for worsening symptoms.

Yes — dehydration can cause a mild to moderate rise in serum amylase. Reduced kidney perfusion and hemoconcentration lower amylase clearance, producing a transient elevation. Marked or persistent increases more often indicate pancreatic or other pathology (pancreatitis, salivary disease, renal failure). Rehydrate and reassess; if levels stay high or you have abdominal pain or vomiting, seek medical evaluation.
