Alcohol(Serum)

Alcohol serum in a blood test refers to the concentration of ethanol measured in the blood’s liquid component (serum). It quantifies intoxication for clinical, forensic or legal purposes and is reported in mg/dL or mmol/L. Serum levels are usually slightly higher than whole-blood values. Results must be interpreted considering timing of drinking, individual metabolism, medications, and sample handling.

A positive alcohol test means ethanol or its metabolites were detected in a biological sample (breath, blood, urine, saliva or hair) above the test’s cutoff. It indicates recent alcohol use; blood and breath measure current intoxication (legal BAC limits, commonly 0.08% in many jurisdictions), while urine (EtG/EtS) and hair tests detect more recent or longer-term consumption. Some medications or contamination can rarely cause false positives.

No—“alcohol serum” means ethanol measured in blood serum, while BAC (blood alcohol concentration) refers to ethanol in whole blood, reported as a percentage or mg/dL. Serum ethanol is usually about 10–15% higher than whole-blood BAC because serum has more water per volume. They’re related and convertible, but not identical; legal limits use whole-blood BAC.

Normal blood alcohol concentration (BAC) for someone who hasn’t consumed alcohol is 0.0%. Legally acceptable driving limits vary by country—commonly 0.05% (many countries) or 0.08% (US)—but impairment can begin at about 0.02–0.05%. Pregnant people, underage individuals and non-drinkers are advised to maintain 0.0%. Individual factors (weight, sex, medication, food) affect BAC and impairment.

Alcohol-related liver damage is detected by liver function tests: elevated AST and ALT (often AST>ALT with ratio >2), high GGT, raised bilirubin and alkaline phosphatase, low albumin and prolonged INR indicating reduced synthetic function. Complete blood count may show macrocytosis (high MCV) and thrombocytopenia from portal hypertension. Trends and clinical context determine severity; further imaging or specialist review may be needed.

High ethanol levels cause slurred speech, impaired coordination, dizziness, nausea and vomiting, headache and decreased alertness. Mood changes, confusion, memory loss and slowed reaction times occur. Severe intoxication may produce slow or irregular breathing, very low body temperature, seizures, unconsciousness or coma and risk of aspiration. Seek emergency care for severe drowsiness, difficulty breathing, persistent vomiting, or loss of consciousness.
