Active Vitamin B12 (Holotranscobalamin)

Holotranscobalamin (holoTC) is the fraction of vitamin B12 bound to transcobalamin II that delivers B12 into cells. It represents the biologically active B12 available for cellular uptake and metabolic use, making it a sensitive early marker of B12 status. Low holoTC levels can indicate cellular B12 deficiency even when total serum B12 appears normal, guiding earlier diagnosis and treatment.

The active B12 (holotranscobalamin) test typically costs about $50–200 in the United States, £40–120 privately in the UK, or roughly ₹800–2,000 in India. Actual price varies by laboratory, city, whether it’s ordered by a clinician or direct-to-consumer, and insurance coverage; some labs bundle it with other B12 markers, which alters out-of-pocket cost.

High active B12 (holotranscobalamin) often reflects recent supplementation or injections but can also occur with liver disease, reduced kidney clearance, blood disorders (myeloproliferative conditions), certain cancers, or assay interference. It’s usually not harmful by itself, but unexplained elevation should prompt clinical review and further tests to identify any underlying cause rather than changing treatment without medical advice.

The HoloTC (holotranscobalamin) test in India measures the biologically active form of vitamin B12 in serum. It can detect early B12 deficiency more sensitively than total B12 and is used when patients have unexplained anemia, neurological symptoms, during pregnancy, or to monitor replacement therapy. It’s a routine diagnostic blood test; results are interpreted against lab-specific reference ranges and clinical findings.

Low holotranscobalamin (holoTC, “active” B12) is not normal and usually indicates reduced available vitamin B12. It can be an early and more specific marker of deficiency even when total B12 appears borderline. Low holoTC warrants further evaluation—measure methylmalonic acid and/or homocysteine, perform clinical assessment, and consider B12 supplementation or referral to a clinician.

Active B12 (holotranscobalamin) is usually measured in pmol/L. Most labs consider levels above about 35 pmol/L normal. Values around 20–35 pmol/L are borderline and may warrant further testing or clinical correlation; levels below approximately 20–25 pmol/L suggest deficiency and typically require evaluation and treatment. Reference ranges can vary slightly by lab and assay.
