A total acid phosphatase test measures the level of the acid phosphatase enzyme in blood serum. It helps detect and monitor conditions that raise enzyme activity—most notably prostate disease and some cancers, as well as bone or liver disorders and hemolysis. Results support diagnosis and treatment monitoring but are less specific than newer tests, so further investigation is often needed.

High acid phosphatase means increased release of this enzyme from tissues. In men it commonly points to prostate problems (prostatitis, benign enlargement, or less often prostate cancer). Elevated levels can also arise from bone disease, liver disorders, certain blood disorders, or recent trauma/procedures. It’s not diagnostic alone—your clinician will correlate symptoms, repeat testing and order specific follow‑ups (PSA, imaging or biopsy) as needed.

The acid phosphatase test—specifically prostatic acid phosphatase (PAP)—can be elevated in prostate cancer and metastatic disease, so it was historically used as a tumor marker. However, it is nonspecific (also rises with benign prostate conditions, bone disease, and other causes) and has largely been replaced by PSA testing; today it’s rarely used except in specific monitoring or research contexts.

Positive for acid phosphatase means the enzyme was detected in the tested sample. In forensic or sexual‑assault testing it commonly indicates presence of seminal fluid (not necessarily sperm). In medical settings, elevated prostatic acid phosphatase can reflect prostate tissue activity or disease. Results should be interpreted in clinical context and followed by confirmatory tests (microscopy, PSA, culture) and clinical evaluation.

Normal serum acid phosphatase (total) varies by laboratory and assay; typical adult reference ranges are about 0–5 U/L (some labs use 0–3.5 U/L or up to ~11 U/L). Prostatic (prostate) acid phosphatase historically is considered normal when below roughly 4–6 U/L. Always interpret results against the laboratory’s reference interval and clinical context, since age, sex and assay method affect values.

Acid phosphatase is a lysosomal enzyme that removes phosphate groups from molecules, supporting cellular metabolism and turnover.
