Acetone Urine (Qualitative) Test

The acetone test in urine uses a dipstick to detect ketone bodies (mainly acetoacetate and acetone) produced when the body breaks down fat for energy. It helps identify ketosis or diabetic ketoacidosis from uncontrolled diabetes, prolonged fasting, low‑carb diets, alcohol misuse, or vomiting. Standard strips don’t reliably measure beta‑hydroxybutyrate, so a positive result—especially with high glucose or illness—needs medical evaluation.

Qualitative tests for acetone include the nitroprusside (Rothera/Legal) test and the iodoform test. Nitroprusside in alkaline medium yields a purple or magenta color indicating ketone bodies (acetoacetate/acetone). The iodoform test (iodine plus NaOH) produces a characteristic yellow crystalline precipitate (iodoform) with methyl ketones such as acetone. These are screening tests; confirmatory assays measure exact levels.

A positive acetone (ketone) test means your body is producing ketones—byproducts of fat breakdown—because it lacks sufficient glucose for energy. Causes include fasting, low‑carb diets, prolonged exercise, vomiting, illness, pregnancy or insulin deficiency in diabetes. Mild ketosis can be normal; if you have diabetes, high blood sugar, or symptoms (nausea, vomiting, abdominal pain, breathlessness, confusion), seek urgent care.

A positive result is fairly specific but not fully sensitive—some bacteria don’t reduce nitrate and short bladder dwell time or contamination can cause false negatives or positives. Clinical correlation and urine culture are recommended.
