17 - Alpha- Hydroxy Progesterone

Elevated 17‑hydroxyprogesterone suggests impaired cortisol synthesis—most commonly 21‑hydroxylase congenital adrenal hyperplasia. In infants it can cause virilization or salt‑wasting; in older children and adults it may cause premature adrenarche, hirsutism, irregular menses, infertility, or reflect adrenal/ovarian tumors. Diagnosis often requires an ACTH stimulation test and genetic testing; treatment typically includes glucocorticoids and endocrine specialist care.

Normal 17‑hydroxyprogesterone (17‑OHP) varies by age, sex and menstrual phase. In adult women it’s roughly 0.2–1.3 ng/mL in the follicular phase and about 0.6–3.3 ng/mL in the luteal phase. In men and postmenopausal women levels are generally <2 ng/mL. Newborns and pregnant women have much higher values; markedly elevated levels suggest congenital adrenal hyperplasia. Reference ranges can vary by lab and assay; check your lab’s report.

In PCOS, 17-OH progesterone (17-hydroxyprogesterone, 17-OHP) is usually normal or only mildly elevated. Markedly increased 17-OHP points toward nonclassical congenital adrenal hyperplasia (NCAH) from 21‑hydroxylase deficiency rather than PCOS. If baseline 17-OHP is borderline, an ACTH stimulation test helps distinguish NCAH. Typical cutoffs: baseline <2 ng/mL makes NCAH unlikely; >6 ng/mL is suggestive.

Low 17‑hydroxyprogesterone means reduced production of an adrenal/sex‑steroid precursor. Causes include suppressed ACTH (from steroid treatment or pituitary failure/secondary adrenal insufficiency), rare enzyme defects (e.g., 17α‑hydroxylase deficiency), or sampling/lab issues. Clinical context—fatigue, blood pressure or pubertal changes—guides further testing (cortisol, ACTH, electrolytes) and endocrine referral for diagnosis and management.

High progesterone isn’t inherently bad—it’s normal in pregnancy and the luteal phase or after progesterone therapy. Unexplained or markedly elevated levels can cause fatigue, mood changes, breast tenderness, bloating, headaches, and may signal adrenal/ovarian conditions or rare tumors. If levels are unexpectedly high or you have concerning symptoms, consult your healthcare provider for evaluation and targeted testing.

High progesterone usually indicates you’ve ovulated and the fertile window is closing. The egg is viable for about 12–48 hours after ovulation, so pregnancy can still occur if sperm were present around ovulation. Once several days into the luteal phase, chances drop. Progesterone rises further in early pregnancy and with supplements, but high levels alone don’t guarantee or prevent pregnancy.
