A temporomandibular (TM) joint X‑ray visualizes the bony components — the mandibular condyle, glenoid fossa and articular eminence — showing alignment, fractures, joint‑space changes, bone erosion, osteoarthritis, bone spurs, ankylosis or dislocation. It detects degenerative changes and structural abnormalities but is limited for soft tissues (articular disc); MRI or CT better assess discs and detailed bone pathology.

Position the patient sitting or standing with the head stabilized. Obtain at least two views: closed‑mouth and open‑mouth (to show condylar translation), using a transcranial or lateral oblique projection for each side. Instruct the patient to bite a spacer for open mouth and keep teeth in occlusion for closed. Use thyroid shielding, correct exposure, and immobilize the head. For disc/soft‑tissue concerns, order MRI or CBCT.

Patient is positioned upright or seated with the side of interest closest to the image receptor in a true lateral position. The midsagittal plane is perpendicular to the receptor and the Frankfurt horizontal plane is parallel to the floor. The head is stabilized; images are taken with mouth closed and then open to assess joint spaces and condylar movement. Immobilization reduces motion artifact.
