Standard radiographic views for the calcaneus are a lateral view and an axial (plantodorsal) calcaneal view, commonly the Harris or Saltzman projection. Additional targeted projections include Broden (oblique) views to evaluate the posterior facet/subtalar joint and AP or oblique foot and ankle radiographs when associated injuries are suspected. CT is preferred for complex fractures.

Standard radiographic assessment for heel pain includes weight-bearing lateral and axial (plantodorsal/Harris–Beath) views of the calcaneus. Additional anteroposterior and oblique foot views, and hindfoot alignment radiographs, may be requested to evaluate fractures, calcaneal spurs, joint changes and alignment. Weight-bearing images are preferred to assess plantar load-bearing anatomy and subtle deformities.

Center on the calcaneus. For a lateral calcaneus view, direct the central ray to the mid‑calcaneus about 1 inch (≈2.5 cm) inferior to the medial malleolus. For the axial (plantodorsal/Harris) view, angle the beam roughly 30–40° cephalad to the foot’s long axis and center at the base of the third metatarsal, through the calcaneus.

A calcaneal (heel) spur is most clearly demonstrated on a lateral weight-bearing radiograph of the foot—commonly called a lateral calcaneal view. This view visualizes plantar and posterior bony spurs. Additional axial (Harris-Beath/hindfoot) projections may be used to better define spur orientation and extent. Weight-bearing images are preferred to assess relation to the heel fat pad and plantar fascia.

Yes, many calcaneus (heel bone) fractures are visible on standard X‑rays, particularly lateral and axial (Harris) views. Look for displacement, comminution and an altered Bohler’s angle. However, subtle or intra‑articular fractures can be missed on plain radiographs; CT is often used for clearer detection and for preoperative or detailed assessment when X‑rays are inconclusive.

The four standard ankle X‑ray views are: AP (anteroposterior) — overall alignment; Mortise — AP with 15–20° internal rotation to open the ankle joint space; Lateral — lateral profile of tibia, talus and calcaneus; and Oblique — angled view to better show the distal fibula, tibiofibular syndesmosis and joint margins.
