X Ray Chest PA View

PA view in chest X-ray means a posteroanterior projection: the patient stands facing the image detector while the X‑ray beam passes from back (posterior) to front (anterior). It’s done upright with full inspiration, placing the heart closer to the detector to reduce magnification and provide a more accurate cardiac size and clearer lung fields compared with AP films; scapulae are moved out of the lung fields.

A PA (posteroanterior) chest X-ray can detect lung infections (pneumonia), consolidation, masses or nodules, atelectasis, pleural effusion, pneumothorax, and signs of heart enlargement or heart failure (cardiomegaly, pulmonary edema). It also shows chronic lung changes (COPD, interstitial disease), rib fractures, calcifications and the position of lines/tubes. Small or early abnormalities may need CT for confirmation.

A normal chest PA shows clear lung fields without focal consolidation, collapse, mass, pleural effusion, or pneumothorax; normal heart size and contour; normal mediastinal and hilar contours; appropriate bronchovascular markings for age; and intact, normally aligned bony structures. Overall there is no radiographic evidence of acute cardiopulmonary disease. Clinical correlation is recommended if symptoms persist.

The four views of a chest X‑ray are posteroanterior (PA), anteroposterior (AP), lateral, and lordotic (apical). PA is the standard erect projection with the X‑ray beam entering posteriorly; AP is used for portable or supine films. Lateral provides depth and posterior-anterior detail, and the lordotic view visualizes the lung apices and clavicles.

A chest PA (posteroanterior) X‑ray is considered safe for most people because it uses a very low radiation dose (around 0.02 mSv), comparable to a few days of background radiation. It’s routinely used and benefits generally outweigh the minimal risk. Extra precautions apply for pregnant patients and children—technicians minimize exposure, use shielding and tailored settings, and avoid unnecessary repeats.

PA chest radiographs are preferred because the X‑ray beam enters the back and exits the front, placing the heart nearer the detector and reducing cardiac magnification. This produces a more accurate heart size and clearer lung detail, with less distortion and better inspiratory effort. AP views (portable/supine) are for immobile patients but often overestimate heart size and reduce diagnostic quality.
