X Ray Both Hindfoot Alignment Long Axial View (Weight Bearing)

The hindfoot alignment view is a weight‑bearing radiograph (Saltzman/long axial) used to assess the relationship between the tibial axis and calcaneus. Normal alignment shows the calcaneal axis colinear with or slightly valgus to the tibial axis (typically 0–5° valgus), with the heel centered under the tibial plafond and no significant varus or valgus deformity.

A weight-bearing X-ray is taken while the patient stands or otherwise supports weight on the limb, so joints and bones are imaged under physiological load. This reveals alignment, joint-space narrowing, collapse or instability that non–weight-bearing films may miss. Commonly used for knees, ankles, feet, hips and spine; single-leg, bilateral or seated loading views can be ordered depending on the clinical question.

The long-axis view of the foot is an imaging plane aligned with the foot’s longitudinal axis, producing a lengthwise image of bones, joints and soft tissues. In ultrasound or radiography it shows structures (e.g., Achilles tendon, plantar fascia) along their fibers to assess continuity, tears, thickening or alignment. The probe or beam is oriented parallel to the structure for optimal lengthwise detail.

A weight-bearing foot X-ray is a radiograph taken while you stand, so the bones, joints, and arches are imaged under normal load. It helps assess alignment, joint space, fractures, arthritis, and deformities (for example flatfoot or Lisfranc injury), guiding treatment or surgery. AP, lateral and oblique views are taken with low radiation; if you cannot stand, non–weight-bearing imaging is used.

A weight-bearing view of the ankle is an X-ray taken while the patient stands (bears weight) to show how bones and joints align under physiological load. Typical views include AP/mortise and lateral images. It reveals joint space, talar shift, syndesmotic widening or subtle instability and fractures that may be missed on non–weight‑bearing films, guiding diagnosis and treatment.

Measure hindfoot alignment with a weight-bearing assessment: have the patient stand and observe the heel relative to the tibial axis (plumb line). Clinically, bisect the calcaneus and draw the tibial shaft line; use a goniometer to record the angle. For confirmation, obtain a weight-bearing hindfoot (Saltzman) radiograph and measure the angle between the tibial and calcaneal axes.
