A Western blot historically served as a confirmatory test for HIV antibodies, but it is no longer recommended as the sole confirmatory test. A positive Western blot usually indicates HIV antibodies, but modern diagnosis follows a lab algorithm: initial antigen/antibody screening, then an HIV‑1/2 differentiation test and, if needed, nucleic acid testing. Western blot can give indeterminate results and requires follow-up.

A Western blot HIV test’s laboratory run time is several hours, but the practical turnaround from blood collection to report usually takes 1 to 3 days in many labs; some facilities may take up to a week depending on workload and shipping. Clinics can often give expedited timing; if faster confirmation is needed, ask about alternative tests (nucleic acid or 4th‑generation assays).

The Western blot was replaced because newer HIV testing algorithms use fourth‑generation antigen/antibody assays and nucleic acid tests that detect infection earlier, are more sensitive and specific, automated, faster, and produce fewer indeterminate results. Western blot’s interpretation is complex, can yield ambiguous patterns, and misses early infections. Modern tests simplify diagnosis, reduce delays, and improve linkage to care.
