Ultrasound Hip

Hip ultrasound can detect fluid or joint effusion, synovitis and bursitis; evaluate tendons, ligaments and muscle tears or strains; identify soft-tissue masses, abscesses or cysts; assess hip stability and developmental dysplasia in infants; and guide joint aspiration or injections. It’s noninvasive and real-time but limited for deep bony, cartilage or intra‑articular detail and is operator-dependent.

A hip ultrasound for adults uses high-frequency sound waves to create real-time images of soft tissues around the hip—muscles, tendons, bursae and joint fluid. It evaluates pain, swelling, tendon tears, bursitis and guides aspirations or injections. The test is noninvasive, radiation-free and portable, but is less effective than MRI for imaging deep bone or complex intra‑articular structures.

Initial X‑ray is best to assess bones and arthritis. For persistent pain or suspected soft‑tissue injury, labral tear, early avascular necrosis or occult fracture, MRI is the most informative. CT is preferred for complex fractures or surgical planning. Ultrasound helps evaluate superficial soft tissues and guide injections. Bone scan can detect stress fractures or infection when other tests are inconclusive.

Do a hip ultrasound in infants with abnormal newborn checks (positive Ortolani/Barlow), asymmetric skin creases, leg‑length discrepancy, breech delivery or family history of developmental dysplasia — ideally within the first 4–6 months while the hip is cartilaginous. In older children and adults it’s used for hip pain to detect effusion, synovitis, bursitis, tendon tears or to guide aspiration and injections.

Hip ultrasound is used to assess infant hips for developmental dysplasia and is most useful in the first months of life. It’s commonly performed from around 4–6 weeks and remains reliable up to about 4–6 months; after bony ossification progresses, radiographs (X‑rays) become the preferred imaging for older infants and toddlers.

Therapeutic ultrasound can give short-term relief for some soft-tissue hip problems (bursitis, tendinopathy), but evidence for lasting benefit is limited and mixed. Diagnostic ultrasound is helpful to detect bursitis, tendon injury, or effusion. Overall it’s generally safe and may be a useful adjunct to exercise and physiotherapy, but shouldn’t replace targeted rehabilitation—discuss options with your clinician.
