Transperineal Ultrasound

A transperineal ultrasound uses a probe over the perineum to image pelvic organs and the pelvic floor. It assesses fetal head position during labor, detects obstetric anal sphincter injury and levator ani defects after birth, evaluates urinary incontinence and pelvic organ prolapse, measures urethral/bladder anatomy, and guides pelvic procedures. It is noninvasive, well-tolerated, and useful in urogynecology and obstetrics.

Perineal ultrasound is a non‑invasive imaging technique using an external probe on the perineum to visualize pelvic floor structures—bladder, urethra, vagina, rectum and supporting muscles. It gives real‑time, dynamic assessment of pelvic organ prolapse, urinary incontinence, childbirth injuries and fistulas without radiation. The exam is painless, quick and often performed during rest, squeeze and Valsalva maneuvers to evaluate function.

Explain the procedure and obtain consent. Position the patient supine with hips flexed and knees apart (lithotomy/semi‑recumbent); ensure bladder status as indicated. Use a low‑frequency curved or linear probe with plenty of gel and a probe cover. Place the probe on the midline perineum, acquire midsagittal and transverse images (pubic symphysis, urethra, vagina, rectum), ask for rest/squeeze/Valsalva for dynamic assessment, and save images.

The three main types are diagnostic ultrasound (imaging to view organs, fetuses, and guide procedures), Doppler ultrasound (measures blood flow and velocity to detect blockages or clots), and therapeutic ultrasound (uses focused ultrasound energy for treatments like physiotherapy, lithotripsy, or tumor ablation). Safety and settings differ by type and clinical use.

A prostate ultrasound, usually transrectal, often causes mild discomfort or pressure when a small probe is inserted into the rectum. You may feel urgency, cramping, or brief sharp twinges. Lubricant and sometimes local anesthetic reduce sensation. The scan itself is short (about 10–20 minutes). If a biopsy is taken, there can be more soreness, bleeding, or temporary pain, but most men tolerate it well.

Transrectal ultrasound (TRUS) is the standard ultrasound for prostate evaluation — it gives high‑resolution imaging and commonly guides biopsies. For biopsy access, transperineal ultrasound is an increasingly preferred alternative with lower infection risk. High‑frequency probes, Doppler, or contrast‑enhanced techniques can improve lesion detection, though multiparametric MRI generally outperforms ultrasound for cancer localization.
