Sputum Culture And Susceptibility

A sputum culture and sensitivity test analyzes mucus coughed up from the lungs to detect and identify bacteria or fungi causing respiratory infections. The lab incubates the sample to grow organisms, identifies the pathogen, and tests which antibiotics effectively inhibit it. Results guide targeted treatment and detect drug resistance; routine results take 24–72 hours, while some organisms need longer.

A positive sputum culture for Mycobacterium tuberculosis is confirmatory for active pulmonary TB and allows drug‑susceptibility testing. Cultures are more sensitive than smear microscopy but can take weeks to grow. A single negative culture does not completely exclude TB—clinical evaluation, chest imaging, molecular tests (NAAT/PCR) and repeat or additional specimens may be needed to establish or rule out diagnosis.

A positive sputum culture means bacteria, fungi, or mycobacteria are present in the lower respiratory tract. Clinicians use the identified organism and antibiotic sensitivity to select targeted therapy, may order chest imaging and repeat cultures, and implement isolation or public-health notification for contagious pathogens (e.g., tuberculosis). Treatment is adjusted and clinical response monitored closely.

Sputum culture sensitivity is variable—commonly about 50–80% for typical bacterial pneumonia and higher (around 80–85%) for mycobacterial (TB) culture. Specificity is generally high, often ≥90% and can approach 98–100% for targeted pathogens like Mycobacterium tuberculosis. Accuracy depends on specimen quality, prior antibiotics, laboratory methods and the organism sought; a negative culture doesn’t definitively rule out infection.

Costs vary by country and provider. Public-health systems often cover sputum culture free, but private lab fees typically range about $30–$200 in the U.S., £10–£60 in the U.K., or ₹300–₹1,500 in India. Prices rise if antibiotic sensitivity, mycobacterial cultures, or rapid molecular tests are added; turnaround time and local pricing also affect the final cost.

A negative sputum culture means no bacteria were grown, but it does not fully rule out infection. Reasons include prior antibiotics, poor sample quality, or organisms not detected by routine culture (viral, atypical bacteria, or mycobacteria). Clinicians will correlate with symptoms and imaging, and may order repeat or alternative tests (PCR, AFB culture, bronchoscopy) or choose observation, supportive care, or targeted treatment.
