RBC Liver Scan

A red blood cell (RBC) scan — also called a tagged RBC scan — is a nuclear medicine test where a patient’s red blood cells are labeled with a small amount of radioactive tracer and imaged with a gamma camera. It’s used chiefly to localize active gastrointestinal bleeding or identify vascular lesions (e.g., hepatic hemangiomas). It’s minimally invasive, sensitive to low-rate bleeding, and involves low radiation exposure.

Yes. Liver disease commonly affects red blood cells by reducing production (through chronic inflammation and nutrient deficiencies such as folate), causing increased destruction or sequestration (hypersplenism from portal hypertension), promoting bleeding-related iron deficiency, and altering RBC shape (spur/target cells) and lifespan. These changes often cause anemia and abnormal RBC indices, needing investigation and treatment.

The best scan depends on the problem. Ultrasound is the usual first-line test for liver disease and screening. For detailed evaluation or suspected tumors, contrast-enhanced MRI (or CT if MRI is unavailable) provides superior lesion characterization. For noninvasive assessment of fibrosis or cirrhosis, transient elastography (FibroScan) is preferred. Clinicians choose based on clinical context and blood tests.

In the liver, red blood cells are present within its blood vessels—particularly the hepatic sinusoids—transported via the portal vein and hepatic artery. They are not liver cells; hepatocytes don’t contain RBCs. Liver macrophages (Kupffer cells) clear aged or damaged RBCs by phagocytosis, so fragments or degraded RBCs may be seen during normal clearance or in hemolytic conditions.
