Mycobacterium Complex - PCR (BAL)

The Mycobacterium TB PCR test is a molecular assay that detects DNA from the Mycobacterium tuberculosis complex in respiratory or tissue samples (sputum, bronchial washings, lymph node aspirates). It gives rapid, sensitive detection compared with culture, identifying active infection within hours to a day. Some PCR platforms also screen for common drug‑resistance markers; a negative result does not fully exclude TB and may need culture and clinical correlation.

A BAL (bronchoalveolar lavage) test for TB collects fluid from the lower airways during bronchoscopy. Sterile saline is instilled and then suctioned to retrieve cells and organisms for microscopy, culture and molecular tests (such as PCR). It’s used when sputum is negative or unobtainable, improving detection of Mycobacterium tuberculosis and enabling drug‑susceptibility testing.

MTB PCR positive means Mycobacterium tuberculosis DNA was detected in a clinical sample by polymerase chain reaction. It indicates the presence of TB bacteria and strongly suggests infection, often active disease, but may occasionally detect nonviable organisms after treatment. Clinical assessment, chest imaging and confirmatory tests (culture and drug-resistance testing) are needed to guide diagnosis and treatment.

TB PCR is not reported with a single “normal range” like blood tests; results are qualitative. Most assays report Mycobacterium tuberculosis DNA as “detected” or “not detected.” Some platforms (e.g., GeneXpert) give semi‑quantitative categories (high/medium/low/very low) or cycle‑threshold values that are assay‑specific. Positive PCR requires clinical correlation and often confirmatory culture or additional testing.

A positive TB PCR indicates detection of Mycobacterium tuberculosis complex DNA in the specimen, suggesting active tuberculosis. It prompts clinical assessment, chest imaging and microbiological confirmation (sputum smear/culture) including drug‑susceptibility testing. Treatment should be started promptly per guidelines, with public‑health notification and contact tracing. Evaluate for extrapulmonary disease and test for HIV to guide management.

PCR stands for Polymerase Chain Reaction. It is a laboratory technique that rapidly amplifies specific DNA sequences by repeated cycles of denaturation, annealing, and extension using a heat-stable DNA polymerase, primers, and nucleotides.
