MR sialography is an MRI technique that noninvasively images salivary glands and their ducts using heavily T2‑weighted sequences to highlight fluid-filled channels. It shows ductal anatomy and pathology—stones, strictures, dilatation, inflammation, or tumors—without duct cannulation or routine contrast. It’s painless and useful for evaluating recurrent salivary swelling, pain, or dry mouth.

Parotid MRI provides high‑resolution soft‑tissue images of gland size, shape and internal architecture, distinguishing solid tumors, cysts, inflammation, abscesses and vascular lesions. It shows lesion location (superficial versus deep lobe), perineural or skull‑base spread, involvement of the facial nerve, and regional lymph nodes. Contrast enhancement and diffusion sequences improve lesion characterization and help plan surgery or biopsy.

Costs vary widely. In India an MRI of the parotid/neck typically costs about ₹2,500–₹10,000 depending on contrast and centre. In the UK it's free on the NHS but private scans run roughly £300–£800. In the US private charges commonly range from about $400–$3,500 (higher at hospitals); contrast, facility type and reporting fees affect the final price—check local providers and insurance.

MRI can detect some salivary gland stones and is useful for visualizing ducts and surrounding soft tissues, especially with MR sialography. However, MRI is less sensitive than ultrasound or non‑contrast CT for small or heavily calcified stones; CT is best for calcifications and ultrasound is often first choice for superficial glands. Choice depends on stone size, location and clinical context.

Sialography costs vary by country and setting. In private US radiology centers it often ranges roughly $150–$700; in the UK it’s usually provided free on the NHS but private scans may cost about £150–£400. In India prices commonly run from about ₹500 to ₹3,000. Final cost depends on facility, imaging type, contrast use and radiologist/reporting fees.

Sialography risks include pain or swelling during or after the procedure, infection, allergic reaction to iodinated contrast, contrast extravasation and ductal perforation or trauma, and exacerbation of sialadenitis. There is also small radiation exposure from imaging. Rare complications can include worsening salivary gland function or transient nerve irritation. Patients with active infection or contrast allergy may be advised to avoid the test.
