Multiparametric MRI (mpMRI) of the prostate is a non-invasive imaging test that combines anatomical and functional MRI sequences—high-resolution T2, diffusion-weighted imaging (DWI) and often dynamic contrast-enhanced (DCE) scans—to detect and characterize prostate lesions. It improves detection of clinically significant cancer, guides targeted biopsies, assists staging and treatment planning, and uses standardized reporting (PI-RADS) to communicate risk.

Prepare by wearing loose, metal-free clothing and leaving jewelry at home. You may be asked to avoid ejaculation for 24–48 hours and to empty your bowels or use a mild enema to reduce rectal gas. If contrast is planned, disclose kidney problems and bring recent eGFR results. Bring prior imaging, arrive on time, and tell staff about implants, allergies or pregnancy; you will need to lie very still.

Multiparametric prostate MRI is reasonably accurate for detecting clinically significant prostate cancer: pooled sensitivity is about 85–90% and specificity roughly 50–75%, with a high negative predictive value (often around 90%). A negative scan makes significant cancer unlikely but does not rule it out entirely. Accuracy depends on scanner quality, imaging protocol and radiologist experience; PI‑RADS scoring helps standardise reports.

Multiparametric MRI (mpMRI) is the preferred prostate exam. It combines high-resolution T2-weighted imaging with diffusion-weighted imaging (DWI) and ADC maps, plus dynamic contrast-enhanced (DCE) sequences when indicated. mpMRI improves detection, localization, staging and MRI-targeted biopsy planning. A 3T scanner gives superior resolution; an endorectal coil may be used at 1.5T. Reports follow the PI-RADS system.

Multiparametric MRI (mpMRI) is better than systematic biopsy alone at detecting clinically significant prostate cancer: it identifies suspicious areas, reduces unnecessary biopsies, and enables targeted sampling. However, mpMRI cannot fully replace biopsy—histological confirmation and grading still require tissue. Current practice is mpMRI first, then targeted (and sometimes systematic) biopsy when lesions are seen.
