MRA Bilateral Renal Arteries

MRA of the renal arteries is a non‑invasive MRI technique that visualizes renal artery anatomy and blood flow to detect stenosis, occlusion, aneurysm, or vascular anomalies. It may use contrast‑enhanced or noncontrast sequences and is used for evaluating secondary hypertension, transplant planning, or suspected renal artery disease. Advantages include no ionizing radiation; limitations include lower spatial resolution than CT and gadolinium risks in severe renal impairment.

Bilateral renal arteries are the pair of blood vessels that branch off the abdominal aorta to supply each kidney with oxygenated blood. Each renal artery enters its kidney at the hilum, dividing into segmental and smaller branches to perfuse renal tissue. Variations (accessory arteries) are common. Their patency is vital for kidney function and blood pressure regulation; stenosis can cause hypertension and renal impairment.

Doctors order an MRA to noninvasively visualize blood vessels and assess blood flow. Indications include evaluating suspected stroke, aneurysm, arteriovenous malformation, carotid or renal artery stenosis, peripheral arterial disease, vascular inflammation, or planning/monitoring stents and grafts. It helps locate blockages, narrowing, malformations, or bleeding risk without catheter angiography, guiding diagnosis, treatment decisions and surgical planning.

Treatment of bilateral renal artery stenosis focuses on blood-pressure control, cardiovascular risk reduction, and revascularization when indicated. Percutaneous transluminal renal angioplasty with stenting is the preferred revascularization for progressive renal dysfunction, refractory hypertension, or recurrent flash pulmonary edema; surgical bypass is rare. ACE inhibitors or ARBs can worsen kidney function and are used cautiously with close creatinine/potassium monitoring. Antiplatelet therapy and statins are advised.

Magnetic resonance angiography (MRA) cost varies by country and setting. In the US, an MRA typically ranges from about $400 to $3,500 out‑of‑pocket (insurance may cut patient cost to $100–$1,000). In the UK it’s usually free on the NHS; private scans cost roughly £200–£700. In India private centers often charge ₹2,000–₹10,000, depending on location and contrast use.

MRA stands for mineralocorticoid receptor antagonist, a drug class that blocks aldosterone’s effects. In kidney disease MRAs (e.g., spironolactone, eplerenone, finerenone) reduce proteinuria, inflammation and fibrosis and can slow chronic kidney disease progression. They raise the risk of hyperkalemia and may lower eGFR, so treatment requires regular monitoring of renal function and serum potassium.
