Magnetic Resonance Elastography (MRE)

Magnetic resonance elastography (MRE) is a noninvasive MRI technique that measures tissue stiffness by sending low-frequency mechanical vibrations into the body and imaging how shear waves travel through tissues. It generates quantitative stiffness maps (elastograms), most commonly used to detect and stage liver fibrosis. MRE is painless, safe, and generally more accurate than many blood tests or ultrasound for assessing tissue stiffness.

MRE is highly accurate and reproducible for detecting and staging liver fibrosis, especially advanced fibrosis and cirrhosis, with sensitivity and specificity commonly above 90%. It outperforms ultrasound elastography and provides quantitative, whole‑liver stiffness maps. Limitations include reduced accuracy with severe iron overload, motion/ascites, and limited availability and cost. MRE is noninvasive and useful for monitoring progression.

Final cost depends on location, whether it’s added to an MRI, and insurance coverage.

Liver elastography is generally non‑painful and non‑invasive. Most people feel only light pressure or brief vibrations as the probe contacts the skin; the exam is quick (minutes) and requires no needles or sedation. Some may experience mild discomfort if they’re tender, obese, or have a full abdomen. MR elastography can be noisier and involve breath‑holding, but serious complications are rare.

An MRE (magnetic resonance elastography) usually takes about 15–45 minutes, with many liver MREs completed in roughly 30 minutes. Allow extra time for check‑in, changing, positioning and any IV or additional MRI sequences—so expect the appointment to be about 30–60 minutes total. Exact duration depends on the facility, the area scanned, and patient factors such as need for breaks or anxiety management.
