Liquid Based Cytology (HPV)

Liquid-based cytology (LBC) prepares cervical cells for microscopic examination but does not directly detect HPV. LBC can show cellular changes suggestive of HPV (for example, koilocytosis), but definitive diagnosis requires molecular HPV DNA/RNA testing. Many laboratories use the same LBC specimen for HPV tests (co-testing), enabling both cytology assessment and accurate HPV detection from one sample.

A cytology test for HPV — commonly called a Pap smear or cervical cytology — collects cells from the cervix to examine under a microscope for abnormal changes caused by high‑risk HPV types. It detects precancerous or atypical cells so clinicians can monitor, repeat testing, or refer for colposcopy and treatment to prevent cervical cancer. It’s often done alongside HPV DNA testing.

Liquid-based cytology (LBC) is a cervical screening test that examines cells from the cervix to detect abnormal or precancerous changes and early cervical cancer. The same sample can be used for HPV testing to identify high‑risk human papillomavirus strains linked to cervical cancer. LBC improves sample quality, reduces obscuring material, and may also indicate infections or inflammation.

Liquid-based cytology (LBC) is considered better than conventional Pap smear because it produces a more representative, cleaner sample with fewer unsatisfactory slides, improves detection of abnormal cells, and allows HPV testing from the same vial. Cells are preserved in liquid, reducing obscuring blood or mucus and lowering inadequate sample rates, resulting in more reliable results and fewer repeat tests.

If cytology is negative but HPV‑positive, it usually indicates infection without visible cell changes. Management is surveillance: repeat HPV testing and/or cytology in 12 months because most infections clear. Immediate colposcopy is recommended if high‑risk types (especially HPV 16 or 18) are detected or if HPV persists. Immunocompromised patients may need earlier assessment.

Liquid-based cytology (LBC) is more accurate than conventional Pap smears: it reduces inadequate samples and slightly increases detection of abnormalities. Sensitivity for detecting high‑grade cervical lesions is generally reported about 70–90%, with specificity typically around 90–95%, depending on population and laboratory quality. Accuracy improves further when combined with HPV testing; sampling technique and lab processing remain important determinants.
