Insulin Random

An insulin random test measures circulating insulin in a blood sample taken at any time, without fasting. It helps assess insulin production, detect hyperinsulinemia or low insulin, and support evaluation of insulin resistance or unexplained hypoglycaemia. Results must be interpreted with blood glucose and clinical context because insulin levels change with meals, time of day, and laboratory reference ranges.

Random (non‑fasting) insulin levels vary with recent food intake; many labs consider a normal random value roughly under 50–60 µIU/mL. By comparison, fasting insulin reference ranges are typically about 2–20 µIU/mL. Exact normal ranges depend on the laboratory assay and units used, so results should be interpreted in context with clinical findings and discussed with your healthcare provider.

A high random insulin suggests hyperinsulinemia, commonly from insulin resistance (prediabetes, metabolic syndrome, obesity, PCOS) and rarely insulin-secreting tumors. It warrants repeat and additional tests—fasting glucose, HbA1c, fasting insulin or C‑peptide, and oral glucose tolerance—plus physician evaluation. Management focuses on weight loss, diet, exercise, treating underlying causes; seek urgent care if severe hypoglycemia occurs.

High insulin levels are commonly seen in people with PCOS because insulin resistance can increase androgen production and disrupt ovulation. However, high insulin alone doesn’t diagnose PCOS—other conditions (obesity, metabolic syndrome, type 2 diabetes) can cause hyperinsulinemia. A good fasting insulin is generally about 2–25 µIU/mL (≈14–174 pmol/L); many healthy, insulin‑sensitive people have fasting levels under 10 µIU/mL. Post‑meal insulin normally rises; persistent fasting insulin above ~25 µIU/mL suggests insulin resistance and should prompt medical evaluation.
