Desmoglein I

Desmoglein 1 is a cadherin family protein in desmosomes that mediates strong cell to cell adhesion between keratinocytes, especially in the superficial epidermis. It maintains skin barrier integrity, mechanical resilience and promotes epidermal differentiation. Loss, autoantibody targeting or proteolytic cleavage of desmoglein 1 causes blistering and impaired barrier function, as seen in pemphigus foliaceus and staphylococcal scalded skin syndrome.

A positive desmoglein‑1 result means the immune system has produced antibodies against desmoglein‑1, a skin protein that helps keratinocytes stick together. This finding supports a diagnosis of autoimmune blistering diseases—most classically pemphigus foliaceus—causing superficial blisters and erosions. It must be interpreted with clinical exam and biopsy; antibody levels can help confirm diagnosis and monitor disease activity.

Pemphigus vulgaris primarily targets desmoglein‑3 (Dsg3). The mucosal‑dominant form shows mainly anti‑Dsg3 antibodies causing oral lesions; mucocutaneous PV usually has autoantibodies to both Dsg3 and desmoglein‑1 (Dsg1), producing skin and mucosal blisters. Isolated anti‑Dsg1 without Dsg3 indicates pemphigus foliaceus. Diagnosis uses biopsy and serologic testing; treatment involves systemic immunosuppression, typically corticosteroids and steroid‑sparing agents.

Desmogleins are cadherin-family adhesion proteins in desmosomes that maintain skin, mucous membrane and heart muscle integrity. Clinically, tests for autoantibodies to desmoglein (Dsg1, Dsg3) help diagnose autoimmune blistering diseases such as pemphigus vulgaris and foliaceus. High‑dose oral corticosteroids (eg, prednisone/prednisolone) remain the initial mainstay where rituximab is unavailable or contraindicated, often combined with steroid‑sparing immunosuppressants. Rituximab leads to higher remission rates, reduced steroid exposure, and requires specialist supervision.
