Creatinine Clearence Test

Normal creatinine clearance in adults is roughly 90–120 mL/min, though reference ranges vary with sex, body size and decline with age. Values below about 60 mL/min generally indicate reduced kidney function (chronic kidney disease), while higher values may reflect increased muscle mass, pregnancy, or hyperfiltration. Always interpret results in clinical context.

The measured creatinine clearance (mCrCl) from a timed urine collection: CrCl = (U_cr × V) / P_cr, where U_cr = urine creatinine concentration, V = urine flow rate (mL/min), P_cr = plasma/serum creatinine; result in mL/min. The Cockcroft–Gault estimate: CrCl = [(140 − age) × weight(kg) × (0.85 if female)] / (72 × serum creatinine [mg/dL]). Use ideal/body weight as clinically appropriate.

Normal pregnancy creatinine is lower than usual—about 0.4–0.8 mg/dL. Be concerned if levels exceed ~0.8–1.0 mg/dL, if there’s a sustained rise from your baseline, or if creatinine reaches >1.1 mg/dL. Seek prompt medical review for rising creatinine, reduced urine output, sudden swelling, high blood pressure, severe headache, or visual changes—these can signal kidney injury or preeclampsia.

Low creatinine clearance indicates reduced kidney filtration and impaired renal function. Waste products and drugs can accumulate, causing fluid and electrolyte imbalances, high blood pressure, anemia, and bone‑mineral problems. It may signal acute or chronic kidney disease and often requires medication dose adjustment, further testing and specialist care; severely low values can lead to uremia and need for dialysis.

There isn’t a single serum creatinine number that defines kidney failure — results depend on age, sex and muscle mass. Kidney failure (end‑stage renal disease) is defined by an eGFR under 15 mL/min/1.73 m². As a guide, markedly raised creatinine (for example >4 mg/dL) indicates severe impairment, and levels above ~10–12 mg/dL are commonly seen in established kidney failure, but interpretation requires clinical context.

High creatinine clearance means the kidneys are filtering creatinine faster than normal. It can reflect benign states (pregnancy, high muscle mass, high protein intake) or glomerular hyperfiltration seen early in diabetes, with certain medications, or from lab error. Alone it isn’t always harmful, but persistent elevation warrants evaluation to identify reversible causes and monitor kidney function.
