Neutralizing antibodies are measured from a blood sample using assays that test whether antibodies block viral entry. Gold-standard tests are live-virus plaque reduction neutralization tests (PRNT) and pseudovirus neutralization assays; surrogate neutralization tests (competitive ELISA) provide simpler, widely available alternatives. Binding antibody tests (ELISA/CLIA) suggest exposure but don’t confirm neutralization. These tests are done in specialized labs; discuss results with your clinician.

The anti‑N antibody test detects antibodies to the SARS‑CoV‑2 nucleocapsid (N) protein, indicating prior natural infection. It measures IgG/IgM responses and is used for serosurveys and assessing past exposure; it does not reliably diagnose acute infection. Because most vaccines target the spike protein, anti‑N positivity usually reflects infection rather than vaccination. Antibody levels can wane over months.

COVID antibody tests (serology) detect immune proteins—usually IgM and IgG—in blood to show past infection or response to vaccination. Common methods include ELISA, chemiluminescent assays, and rapid lateral‑flow (finger‑prick) tests. They do not diagnose active infection. Samples are venous or capillary; results range from minutes (rapid) to days (lab). Accuracy varies; consult a clinician for interpretation.

After SARS-CoV-2 infection or vaccination, IgM antibodies typically appear within 1–3 weeks and decline after 2–3 months. IgG antibodies arise by 2–4 weeks and are often detectable for at least 6–12 months; levels fall over time but memory B cells can persist longer, helping protect even when circulating antibodies decrease. Duration varies by age, illness severity, and vaccination.

A virus neutralization test measures functional antibodies that block a virus from infecting cells. Patient serum is mixed with live virus or a non-replicating pseudovirus, then applied to cell cultures; reduction in plaques, cytopathic effect, or reporter signal indicates neutralization. It's the gold-standard for assessing protective immunity and vaccine response, but live-virus tests require high-level biosafety facilities.

To check COVID antibody levels, get a blood-based serology test (finger-prick or venous) from an accredited lab or healthcare provider. Ask for a quantitative assay measuring IgG against spike protein about 2 to 6 weeks after vaccination or infection. Results give antibody titers but don’t guarantee protection; interpretation should be done with your clinician, who can consider timing, test type, and clinical context.
