Calcium- Total

Total calcium is the blood measurement of all circulating calcium: ionized (free) plus protein‑bound (mainly to albumin) and complexed forms. Typical adult reference ranges are about 8.4–10.2 mg/dL (2.1–2.6 mmol/L). Because much calcium binds to albumin, low or high albumin alters total calcium—ionized calcium or an albumin‑corrected calcium is used when albumin is abnormal.

High total calcium (hypercalcemia) can indicate primary hyperparathyroidism, cancer, excess vitamin D or calcium, certain drugs, dehydration, or granulomatous disease. Symptoms include fatigue, nausea, constipation, polyuria, kidney stones, bone pain and confusion. Repeat tests, check ionized calcium, parathyroid hormone, kidney function and vitamin D, and review medications. Seek urgent care for severe symptoms like vomiting, profound weakness, confusion or irregular heartbeat.

Normal blood calcium (total serum calcium) is about 8.5–10.2 mg/dL (2.12–2.55 mmol/L). Ionized (free) calcium—the biologically active fraction—typically runs about 4.6–5.3 mg/dL (1.15–1.33 mmol/L). Reference ranges vary by lab and are affected by albumin, medications and health status; mild deviations may prompt repeat testing or evaluation of parathyroid and vitamin D status.

Calcium total tablets are used to prevent and treat calcium deficiency and support bone and tooth health. They help maintain normal muscle function, nerve transmission, and blood clotting. Commonly prescribed for osteoporosis prevention and treatment, during pregnancy or lactation, and for hypocalcemia. They’re often combined with vitamin D to improve absorption and reduce fracture risk.

Low calcium (hypocalcemia) causes numbness or tingling around the mouth and in fingers, muscle cramps, twitching or painful spasms (tetany), and in severe cases seizures. It can also cause fatigue, confusion and heart rhythm changes such as prolonged QT and arrhythmias. Long-term deficiency weakens bones, increasing fracture and osteoporosis risk. Common causes include vitamin D deficiency, low parathyroid hormone and kidney disease.

Optimal coronary artery calcium (CAC) is 0 at any age, indicating no detectable calcified plaque. CAC is usually categorized: 1–10 minimal, 11–100 mild, 101–400 moderate, and >400 extensive. Younger adults typically have 0–10; mild scores become more common after age 50 and moderate-to-high scores rise with advancing age. Higher scores signal greater atherosclerosis and cardiovascular risk.
